One row per household: the name as it goes on the envelope, then street, city, state and ZIP. Start from the free card list template, or use the list you have: a workbook in Excel, a sheet in Google Sheets, or a contacts export saved as a CSV or vCard (.vcf) file, such as one from Google Contacts or Outlook.
The Contacts app prints labels by itself: select the contacts or a list, choose File > Print, then Mailing Labels in the Style menu, and under Layout a page type such as Avery Standard (Apple's guide). To print them here instead, select the contacts, choose File > Export > Export vCard, then open the .vcf file above (Apple's steps).
Put the name the way it reads on the envelope, “The Okafor Family” or “Ruth & Daniel Alvarez”. How to address them.
The number is on the box. 5160 suits most cards, and 8160 is the same layout for inkjet printers. Which labels to buy.
Choose Actual size in the print dialog. Print one page on plain paper first and hold it over a label sheet against a window.

Printers differ: some print on the side facing up in the tray, others on the side facing down. Mark an X on a plain sheet, print once and look before you load labels.
Hold a plain-paper print over a 5160 sheet against a window. If it sits off, calibrate once and save the correction for that printer.

Word does it with a mail merge: Mailings > Start Mail Merge, then Labels, with your label vendor and product number in Label Options, and your list as the recipients (Microsoft's steps). Keep the list in a spreadsheet you can use again next year, print on the sheet your box names, at 100%, after one test on plain paper. Print a few sheets at a time, so a jam costs a sheet, not the box.
