The workbook you were merging works here: column names in a header row, one address per row. The names it knows.

Printers differ: some print on the side facing up in the tray, others on the side facing down. Mark an X on a plain sheet, print once and look before you load labels.
Hold a plain-paper print over a 5160 sheet against a window. If it sits off, calibrate once and save the correction for that printer.
Rows unticked or filtered out of the recipient list are skipped, and Current record merges only the one on screen. Check Mailings > Edit Recipient List, then use Finish & Merge > Edit Individual Documents and choose All (fix 4).
The merge is set up as letters, and Word prints each letter on its own sheet. Start again with Mailings > Start Mail Merge > Labels (fix 3).
The labels after the first have no «Next Record» field, so every one repeats the same row. Keep only the first label and choose Update Labels (fix 2).
Let Word add it: Mailings > Update Labels puts «Next Record» in front of every label after the first. To add one yourself, use Mailings > Rules > Next Record (Microsoft's steps). Fields typed by hand between « and » don't work.
The document isn't connected to a list yet. Choose Mailings > Select Recipients and pick your list first (Microsoft's note).
