Avery® 5011 place cards are 1-7/16 × 3-3/4 in (2-7/8 × 3-3/4 in open), 6 to a US Letter sheet in 2 columns and 3 rows. The first card starts 1-1/2 in from the top edge and 0.325 in from the left edge; columns are 0.35 in apart and rows are 1/8 in apart. Each card folds across its middle, and its top half prints upside down so both sides read upright.
The blank PDF has every card's outline and its fold, dashed, for checking the layout on plain paper.

Why the top half is upside down
A place card stands on the table with the fold on top, so each half is read from its own side. The PDF turns the upper half 180° about its center: fold along the middle and both read upright.

Printers differ: some print on the side facing up in the tray, others on the side facing down. Mark an X on a plain sheet, print once and look before you load the card sheets.
Hold a plain-paper print over a 5011 sheet against a window. If it sits off, calibrate once and save the correction for that printer.

On purpose. The card stands with the fold on top, so the half above the fold on the sheet ends up facing the other side of the table. Printed upside down, it reads upright from there. Fold along the score with the printed side out, and keep the text 0.15 in or more from the fold, as the PDF does.
No. Both faces print on one side of the sheet: the front below the fold, the back above it, turned. Avery's product page says the same: print on one side of the sheet, then fold along the pre-scored line.
Open your guest list in the editor or paste its cells: one card per row, the name large and bold on the first line, a table number or a note under it, on both faces. Six guests fill a sheet.
Yes. In the editor's Style settings, set the back of the card to Blank: only the half below the fold prints. The front is 1-7/16 in tall, so a name still has the room it needs.

Yes, to about two thousandths of an inch: Avery's own template for 35701 sets its columns 0.1 to 0.15 pt from 5011's, far less than a printer drifts, though Avery doesn't list the two as sharing a template. The gold border is printed on the card and drawn in neither template, so keep your lines short of it, and print the blank PDF on plain paper to hold over a 35701 sheet first.
Yes. Set "Start at card" to the first card left: card 1 is the top left, and the count runs across each row. The cards before it stay blank.
Check that the print dialog says 100% (Actual size), not "Fit to page". If the print still sits a little off, print the test sheet on the calibration page: it draws every card's perforation and its fold, so you can save a correction for your printer.
You don't need them: this page makes a PDF with the top half already turned. If you set the cards up yourself, the half above the fold has to print upside down for the card to read right once it is folded.
Paste your guest list or open a CSV or Excel file: one card per row, the name on both sides, on as many 5011 sheets as it takes.
